Feedback QR Code

Hear from guests. While it counts.

Feedback QR Code

Made from your picture, not pasted on top

A feedback QR code opens your feedback form or survey, so a guest, patient or visitor can answer while the visit is still fresh. AriaQR makes it from your logo or a picture, so the card at the desk looks like your hotel, clinic or shop and not a plain square. The code is dynamic: you can change the form after printing, scans are counted, and every code is tested on real decoders under real-world conditions before you download it.

Where it goes

Where people use a Feedback QR code

Hotels and guest rooms

A card on the nightstand or at check-out that opens your guest survey, so you hear about the stay before it turns up in a public review.

Clinics and practices

On the reception desk or the after-visit sheet, linking to a short patient survey.

Retail and showrooms

A stand at the till or a code on the receipt. A QR code for feedback lets customers say what went well or badly without a long form.

Events and conferences

On the exit door, the badge or the programme, linking to a session or event survey while people are still in the building.

How it works

How to make a Feedback QR code with your logo

  1. 1

    Prepare your form or survey

Build it in the tool you already use and copy its public link. Any web form or survey page works.

  2. 2

Upload your logo or a picture

Your logo, a photo of the venue or the artwork from your signage. PNG, JPEG or WebP.

  3. 3

Paste the link, then scan it

The form opens on a website link. Paste your survey address. Every code is tested before you see it; scan it with your phone and it should open the form.

  4. 4

    Download and print

Download the full-size file. Choose small print for cards and receipts down to about 1.5 cm; standard works from 4 cm and up.

Design tips

Getting the design right

  • Keep the form short

Three or four questions that fit one phone screen get far more answers than a long survey. Test the form on a phone before you print.

  • Ask at the right moment

Put the code where the experience ends: the check-out desk, the exit door, the end of the appointment. People answer more when it is fresh.

  • Tell them what it is for

A line such as “Tell us how we did, it takes one minute” sets expectations. Say who reads the answers if you can.

Questions

Feedback QR code questions

How does a feedback QR code work?

The code opens the link to your feedback form or survey when someone scans it with their phone camera. They answer on their own phone, with no app to install.

What kind of form can I link to?

Any form or survey with a public web address. You build it in the tool you already use, then paste the link into the generator.

Can I put my logo on a feedback QR code?

Yes, and more than a logo in the middle: AriaQR turns the whole picture into the code. Upload your logo or a photo, paste your form link, and you get a branded code that is tested before download.

Can I see how many people scan it?

Yes. Scans are counted, so you can compare how often the card at each desk, room or event is used.

Can I change the form after the cards are printed?

Yes. The code is dynamic, so you can point it at a new form or survey without reprinting.
